New methodology and tool set deliver and support complex population health strategies to improve patient health, drive value and measure success
Bainbridge Island, Wash., September 13, 2016—Health Systems informatics (HSi), a healthcare consulting and solutions firm, today announced new strategic services and advanced analytic capabilities for their Population Health Management Service platform.
Population health management is tied to an increasing number of government regulations and quality initiatives, pushing healthcare organizations to expedite efforts in managing the overall health of populations to reduce admissions, drive down cost and improve patient outcomes. Healthcare leadership is challenged to build a multi-faceted strategy that touches every location across the patient’s continuum of care and all aspects of healthcare services—processes, technology, staff and patients. The HSi Population Health Management Service platform helps hospitals and health systems build and deploy an effective program to achieve the highest standard of patient care, staff satisfaction and financial outcomes.
“Most hospitals are in the early stages of leveraging healthcare IT and data to facilitate population health management, and are frequently finding the initiative overwhelming to embrace with limited resources and expertise,” says Paula Kyburz, Senior Project Director of HSi. “With our extensive experience in healthcare IT consulting, EHR systems, and analytics, HSi is providing guidance and best practice assistance to hospital leadership to build and support a successful population health strategy.”
HSi’s Population Health Management Service provides end-to-end consulting services with a methodology and tool set to develop an enduring program, along with its implementation and analysis. Partnering with some of the most prestigious healthcare organizations in the country, the proven methodology consists of four key components: Define, Assess, Implement and Measure. These services empower hospitals to develop and implement a unified vision for their organization, including:
Strategic Services (Define) – work with leadership to outline program development, governance structures, success measures and expected outcomes, including value management
Assessment and Prioritization (Assess) – conduct a population health management readiness assessment; and, system and process gap analysis to understand the people, process and technology required to build an effective program
Program Management (Implement) – provide certified-application resources and business intelligence and data warehouse experts to lead program implementation and change management
Analytics Build and Testing (Measure) – advanced analytics expertise to gain data-driven insights to improve outcomes in quality, patient engagement, care coordination and fiscal responsibility
“With over 20 years of experience in healthcare IT consulting and data analytics services, it was a natural progression to partner with our clients in their population health program development endeavor,” says Stephanie Hojan, Vice President of HSi. “Population health management is extremely complex and requires full adoption by hospital leadership and staff—it is a paradigm shift—it needs to be treated as much more than a standalone project. HSi understands every organization has a unique set of deliverables and desired outcomes and we help develop programs critical to each organization’s success.”
Leveraging EHR population health and accountable care management framework, HSi’s outcomes-based approach uses best practices and advanced analytics to help healthcare organizations aggregate and segment population data, understand risk, make data-driven clinical decisions and proactively manage patient relationships and community integration.
